Issues with filling up the DTC Form T2201

Did you have issue filling up the Disability Tax Credit application form (T2201)? If yes, or if you have any advice for others going through this process please share.

The form makes it sound like the person has to be almost dying to qualify...

My Disability Tax Credit application wasn't approved by the CRA and i suspect the reason is that my doctor, didn't write enough details about my condition.
I think it the form needs to be improved to make easier for doctors to fill them out. Plus my doctor charged me for filling this form is that normal? Why doesn't OHIP cover this?

I don't one need a professional to apply for disability tax credit.
it is a very simple form that you don't even fill out, and then you mail it in, they tell you if you're qualified or not.. and then you do T1-Adjusts for as many years as they allow for your disability
if a person has a legit disability and there are medical expenses associated with this disability that the taxpayer can proove,
he/she will be approved.
